Understanding perimenopause and menopause

Perimenopause is the transition before menopause, when hormone levels fluctuate and periods may change. Menopause is reached after 12 months without a period when hormonal contraception is not affecting the bleeding pattern. It most commonly occurs between 45 and 55, although it can happen earlier.

For otherwise healthy people aged 45 or over with typical symptoms, NICE advises that perimenopause and menopause can often be identified clinically without routine laboratory testing.

Symptoms can affect far more than periods

Symptoms differ from person to person and can affect daily life, relationships and work. They may include changes to periods, hot flushes, night sweats, sleep problems, mood changes, anxiety, low mood, memory or concentration difficulties, palpitations, headaches, joint or muscle aches, vaginal dryness, urinary symptoms, reduced libido, skin changes and hair thinning.

What support is available?

Management is individual. Depending on symptoms and medical history, discussions may include lifestyle measures, cognitive behavioural therapy for some menopause-associated symptoms, HRT, non-hormonal options, vaginal moisturisers or lubricants, and treatment for genitourinary symptoms. HRT is not right for everyone, and the benefits and risks depend on the person and the type of HRT.

When should you seek medical advice?

Seek advice if symptoms are affecting your quality of life, you are unsure whether menopause explains them, symptoms occur before age 45, or you want to discuss treatment. Any vaginal bleeding after menopause should be checked by a GP. New or severe symptoms should not automatically be attributed to menopause.
